Connie Eefsting, LPN, CMCN, serves as our Client Nursing Coordinator.

Connie's nursing know-how, her vast experience, and her commitment to providing quality care as a Nurse Care Manager make her an invaluable part of our team. Connie works closely with our clients and their families, caregivers, and case managers to assist them in navigating the “elder care maze”. Connie ensures that each client’s “Plan of Care” adapts to meet the client’s individual needs while maintaining quality of life, safety, and independence.

Connie continually strives to enhance her nursing skills. With her passion for life-long learning, Connie earned her CMCN (Certified Managed Care Nurse) certification in July of 2008. Connie is an advocate of in-home care options and views her role as that of an educator in addition to a Nurse Care Manager.

Connie enjoys learning, knitting, camping and spending time with her family.
